How to prevent one off designs/implementations from happening?

When a new requirement is added to the implementation that is not a use case in the living style guide, how should this be handled? Should the designer be involved?

I think in most of the cases a designer should be involved, unless the new requirement is super straight forward. Usually designers have a good handle on the current state of requirements, and will have more contextual information about the requirement, which could potentially be a entire new design pattern that will affect current and future implementations, preventing the one off design situation. Additionally, if applicable, designers can be tasked to translate this new requirement into the design documentation that goes in the living style guide, so their assistance can be helpful for the task at hand as well as for future development that relates to the new requirement.
